Every Denied Claim Is Money You Have Already Earned. We Get It Back.

The average practice loses 5 to 10 percent of annual revenue to unresolved claim denials. Our Enhanced Claim Resolution team identifies, appeals, and recovers those payments — with a documented 94 percent success rate on submitted appeals.

Our Denial Management Process

Medical claim denial management solutions

Denial Tracking

We start by identifying and organizing all denied claims. It involves reviewing electronic remittance advices (ERAs) and Explanation of Benefits (EOBs) to identify denials and understand their causes.

Data Processing Analytics

Root Cause Analysis

Once we categorize the denials, we conduct a detailed analysis to find the reasons behind them. It includes checking coding accuracy, verifying patient eligibility, and ensuring proper medical necessity documentation.

Common Denial Reasons and What They Mean

Most denials fall into a small number of categories. Naming the reason precisely is what decides whether a claim can be appealed or simply has to be corrected and resubmitted.

CO-16: claim lacks information

A required field is missing or invalid. Usually fixable the same day once the missing detail is identified.

CO-197: precertification absent

Care was delivered without an authorisation the payer required. Often recoverable, but only with the clinical justification attached.

CO-22: care may be covered by another payer

A coordination-of-benefits problem. The correct primary payer has to be established before anything else can happen.

CO-29: the filing time limit has expired

The hardest denial to overturn, and always a process signal. Something upstream is taking too long.

CO-50: not deemed a medical necessity

A clinical decision, answered with documentation rather than coding. It requires the treating provider notes.

CO-97 and CO-18: bundled or duplicate

The service was combined into another code, or the claim looks like a duplicate. Correct coding and modifier use prevent most of these.

Tired of Fixing the Same Denials Twice?

We trace every denial back to the front-end issue that caused it and close the loop.

Stop Repeat Denials

Denial Prevention Checklist

Recovering a denied claim costs far more than getting it right the first time. These are the checks that stop denials before they are submitted.

Verify eligibility first

Confirm active coverage, plan type and any deductible position before the appointment, not after the claim comes back.

Confirm authorisation up front

Track which procedures and which payers require prior authorisation, and file it before the service is delivered.

Check the coding before submission

Diagnosis-to-procedure matching, modifier use and NCCI edits reviewed by a certified coder, not just accepted from a billing system.

Watch the filing clock

Timely filing limits vary by payer. The only reliable defence is knowing each payer limit and working to it.

Document medical necessity

If the notes do not support the code, the claim is at risk no matter how cleanly it was submitted.

Work denials in days, not weeks

Appeal windows close. The sooner a denial is categorised, the more options remain open.

The Numbers We Track

Denial management is only as good as the reporting behind it. Our working reports centre on four figures.

Denial rate

Denied claims as a share of claims submitted, tracked by payer and by specialty so the pattern is visible rather than assumed.

Appeal win rate

How often an appeal actually recovers payment, which shows where appeals are worth the effort and where they are not.

Days to resolution

The average time from denial to payment. This is the number that governs cash flow.

A/R over 90 days

Claims ageing past the point of easy recovery, and the earliest warning that revenue is drifting towards uncollectable.

Want the mechanics in more detail? Our guides cover denial management in healthcare, automating denial management, the difference between denials and rejections, and turning lost claims back into revenue.

Denial Management Is Not the Same as A/R Follow-Up

The two are often bundled together, and that is usually where revenue quietly leaks. A/R follow-up chases claims that are simply unpaid: the payer has not answered yet. Denial management starts from an explicit refusal, with a reason code attached and a clock running. They need different skills and they run to different deadlines.

A follow-up queue is measured in days since submission. A denial queue is measured against the payer appeal window, which can be as short as a few weeks. Treating a denial as just another unpaid claim is how practices end up appealing claims they can no longer appeal.

If You Switch to Us: The First 90 Days

Taking over denial management is not a switch you flip. Work already in flight still has to be worked, and the causes behind the denials have to be fixed at the same time. Here is the sequence we follow.

Days 1 to 14: baseline

We pull six to twelve months of claim data and establish the current denial rate, the mix of reason codes, and where the worst of it sits by payer and by specialty. Until that is measured, everything else is guesswork.

Days 15 to 30: work the backlog

The existing denials are aged and sorted. Claims still inside their appeal window are worked first, because those are the ones with money still on the table.

Days 31 to 60: fix the front end

Most denials are created before the claim is ever submitted. Eligibility checks, prior authorisation and coding review are tightened where the baseline shows the failures actually happen.

Days 61 to 90: reporting rhythm

Weekly denial reports, a monthly payer scorecard and a quarterly prevention review. The point is that the numbers become something you act on rather than something you receive.

You do not have to move systems to start. We work inside your practice management system or inside ours, and you keep visibility and ownership of your data throughout.
